We are committed to providing medical care that is affordable for you. Payment and/or insurance co-pays are due when services are rendered.
When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know.
We will submit all insurance claims for you. We will attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance plan, “out-of-network” fees/costs may apply. Please verify that we are participating with your insurance carrier prior to your appointment.
